Use this form to register a SYSTEM ADD-ON to an EXISTING SYSTEM at the SAME PHYSICAL ADDRESS. This form is only for the MAX Series of lights.

Warranty registration does not apply to any previous series of lights and opportunity to register those systems for previous warranties has expired.
